Supporting La Crosse, Vernon, Monroe,
Richland, Crawford and Grant Counties

History

In 1974, CCLS opened our first program in La Crosse to begin serving the Western Region. In 1978, we opened our first Jackson County program. Services continue to be provided today in this CBRF. The first Trempealeau County program began in 1980, and in 1987, we opened an Adult Family Home in Monroe County. We began providing services in Vernon County in 1998.

Family Care in La Crosse set the stage for many additional services. The recreation program originated in April 2002. In addition, Family Care allowed us the opportunity to open five more Adult Family Homes in La Crosse County.

We currently provide services to more than 150 individuals with disabilities in Vernon, La Crosse, Monroe, Juneau, Jackson, Trempealeau and Buffalo Counties. Western Region provides the following residential programs: Community Based Residential Facilities, one and two bed Adult Family Homes, three to four bed Adult Family Homes, live-by apartment programs and 24-hour support programs. In addition, our quality staff of more than 140 also provides day services, daily living skills training, supportive home care, respite and recreational services.

Individualized Services and Supports

  • Unique recreation program which provides clients with opportunities for friendships to develop naturally, for learning to meet and develop social needs and skills, and for enjoying their favorite pastimes as well as discovering new ones.
  • Individualized services that support clients in Adult Family Homes, Community Based Residential Facilities, two-person programs, apartment settings, Independent Living Programs, live-by supports and supportive home care programs.
  • Person-centered supports that encourage independence in personal cares, community integration, daily living skills, socialization, health monitoring, paraprofessional counseling, money management, meal preparation, transportation, medication assistance, recreation and leisure activity skills and mobility training.
  • Day Services are also available.
  • CCLS is able to expand services and supports to neighboring counties.

Individuals Supported

  • Currently support children and adults with developmental disabilities and/or physical disabilities, mental disorders, traumatic brain injury and frail elderly.
  • Diagnoses vary and can include: Down Syndrome, mild/moderate/severe cognitive disorder, epilepsy, autism, brain injury, quadriplegia, paraplegia, cerebral palsy, varying mental health disorders, cerebral vascular accident (stroke), dementia, physical abnormalities, attention deficit disorder, learning disability, blindness, contractures and other conditions.
  • Expanding supports for children with special needs, mental disorders, substance abuse and frail elderly.
