Supporting Dane County
History
On October 8, 1973, CCLS opened its first group home for eight women with developmental disabilities who had spent the last 20 years of their lives in institutions. The home was located on Mifflin Street in Madison’s inner city.
In the years since 1973, the number of programs for people with developmental disabilities has grown throughout Dane County. We are now supporting more than 80 people, 10 of whom are receiving services through the traditional Adult Family Home model while the rest are living in their own apartments or duplexes.
Dane County prides itself on being in the forefront when providing support to adults with developmental disabilities. From 1996 through 2004, Dane County participated in a systems redesign for adults with developmental disabilities based on the principals of Self-Determination. Throughout those nine years, Dane County engaged in a careful planning and learning effort aimed at strengthening the public system of services and supports, while building upon the current and potential strengths of its communities. As of January 2005, more than 1,100 adults with developmental disabilities are accessing services provided through that redesigned system. CCLS officially converted from the Supported Home Contract system to Self-Directed Supports on July 1, 2004.
Individualized Services and Supports
- Individualized services that support clients in Adult Family Homes and Self-Directed Supports based on person-centered planning in which the individuals reside in their own apartments or duplexes.
- Clients receive services that are unique, flexible and chosen by the clients and their circle of support.
- Unique Clubhouse concept for clients to meet with peers, develop and maintain friendships, receive staff support, and engage in their choice of recreational and community activities.
- Person-centered supports that encourage independence in personal cares, community integration, daily living skills, socialization, health monitoring, paraprofessional counseling, money management, meal preparation, transportation, medication assistance, and recreation and leisure activity skills.
Individuals Supported
- Currently support adults with developmental disabilities.
- Expanding the Clubhouse in Dane County to offer services and classes to individuals not currently being served by CCLS.
- Expanding residential and day supports for clients with dual diagnoses of developmental disability and dementia.
