CCLS Services & Supports

Our goal at CCLS is to assist you in maintaining and increasing your independence. In some cases, individual services include learning daily living skills. For individuals who have those skills, our services are suited to fit their needs—for example, help with budgeting/finances or menu planning and nutrition. We encourage social interaction and use community resources to plan activities.

Creative Community Living Services provides person-centered, individualized, community supports. We consider each individual's needs, strengths, and desires and work to build our supports around them. Examples are listed below:

  • Supportive Home Care - Supported apartment living with or without live-in staff
  • Independent Living Programs - Staff offer regular come-in support and on-call availability
  • Adult Family Homes - Three and four bed State licensed facilities. Each is unique.
  • Community Based Residential Facilities - Five to eight bed State licensed facilities
  • Accessible facilities
  • Specialized programs for individuals with traumatic brain injury, dual diagnosis, physical disabilities, medical concerns, behavioral challenges, and other extensive needs
  • Case Management - As designated by certain counties or Care Management Organizations
  • Traumatic Brain Injury Assessment - Upon discharge from acute care
  • Foster Care Sponsor Recruitment and Case Management
  • Personal Care for individuals with physical disabilities and/or medical needs
  • Respite Support for residential providers
  • Come-in supports to the Natural Home environment
  • Assisting in the community
  • Teaching independence in the home and community
  • Offering leisure and recreational support services
  • Providing supportive home care services—assistance with learning and performing typical daily living skills like hygiene and cooking
  • Monitoring health
  • Assisting with medications
  • Providing information and referral to services and resources in the community
  • Day Services

We believe that you should be actively involved in choosing and implementing services and those services should be delivered in the least restrictive environment available.

Your case manager, staff with the Aging and Disability Resource Center and/or our staff will conduct an assessment of your skills and needs. The program and the type of services offered to you are based on that initial assessment. Be assured that as your needs and desires change, our staff will work to ensure you receive the most appropriate supports.
