Navigating Funding
Many Wisconsin counties are not part of Family Care. Your County Human Services is the contact to find out about funding services you need and want. Some of the options that your county case manager or other Human Services staff will discuss with you are Community Options Program, Community Integration Program, Social Security Insurance, private pay, etc.
The Community Options Program helps people who need long-term care to stay in their own homes and communities. Its purpose is to provide cost-effective alternatives to expensive care in institutions and nursing homes. Community Options serves people who need long-term support at the same levels of physical or mental care as nursing home residents, regardless of age or type of disability.
The Community Integration Program helps people with developmental disabilities relocate from State centers and nursing homes back to their communities.
